چکیده

We analyzed surgical results of anterior callosotomy (AC) in 27 patients with intractable symptomatic generalized epilepsy or frontal lobe epilepsy. Excellent outcome (decrease in seizure frequency of greater than 80% or no seizure) was obtained in 59.3%, and 14.8% showed good outcome(decrease in seizure frequency of greater than 50%). There was no significant difference of the results between children and adults. AC was more effective on atypical absence, drop attack, head drop attack and generalized tonic-clonic seizure, but decrease in seizure frequency and severity were also observed in other type of seizures. The extent of AC(anterior 1/2 to 4/5) did not influence seizure reduction. However, patients whose seizures onset before 4 years old had a poor outcome. In postoperative EEG, marked lateralization of diffuse bilaterally synchronous spike and wave (SSW) to unilateral hemisphere implied a better prognosis than that of bilateral independent SW. baba

Stereotactic laser ablation of the splenium for intractable epilepsy☆

Partial or complete corpus callosotomies have been applied, traditionally via open surgical or radiosurgical approaches, for the treatment of epilepsy in patients with multifocal tonic, atonic, or myoclonic seizures. Minimally invasive methods, such as MRI-guided laser interstitial thermal ablation (MTLA), are being employed to functionally remove or ablate seizure foci in the treatment of epil...

Corpus Callosotomy in Pediatric Intractable Epilepsy: Microsurgical Technique Implication and Variation

Medically intractable epilepsy is considered for surgical intervention first for resection of localized area of brain in which cases seizure control is expected generally. Nevertheless, those with non-localizing lesions in the imaging studies as well as multifocal spikes in electroencephalography (EEG) are not candidates for the resective surgery. For palliative intervention, corpus callosotomy...

Management of epilepsy in resource-limited settings: establishing an epileptic surgery program in Southern Iran

Background: The most common treatment modality for epileptic patients is antiepileptic drug (AED) therapy. However, more than 30% of epileptic patient suffers from drug-resistant epilepsy despite the use of appropriate AEDs. Surgery is a common option for these patients. Epilepsy surgery requires sophisticated medical technologies, many of them not available in Iran. The authors of the present ...

The relationship between paired pulse magnetic MEP and surgical prognosis in patients with intractable epilepsy

PURPOSE To assess whether paired pulse magnetic motor evoked potential (MEP) can predict surgical prognosis in patients with intractable epilepsy. METHODS MEP of the unilateral hand muscles were recorded following paired pulse transcranial magnetic stimulation (TMS) of the motor cortex. The interstimulus intervals of paired stimulation were 1-16 ms with a conditioning stimulus that was 90% ac...
